If you prepaid for CLE credit when you registered for our Legal
Issues in Mortgage Servicing Seminar in Chicago, IL, you should
have received a CLE envelope in your registration packet on-site.
If you did not sign-up for CLE credit when you registered, you can
download the documents below (and we will invoice you $25/state when
your completed forms are received at our National Office: USFN,

92780). Please note: Some states require special reporting
forms which we could not post below. If your state utilizes one of
these computerized/special forms, please contact Kelly Mauras
